ඔබ විසින් සකස් කරගත් අයදුම්පතක් (නම, ලිපිනය, දුරකතන අංකය, කැමති පාඨමාලාව (පූර්ණ කාලීන/අර්ධ කාලීන* අධ්‍යාපන සුදුසුකම් ඇතුළත්ව 2018.12.31 වැනි දිනට පෙර යොමු කරන්න.
